Brother Medardus grows up under pious guardianship and enters the Capuchin monastery at Bamberg, renowned for his eloquence. Charged with guarding the sacred relics, he takes custody of a mysterious flask said to contain St. Anthony's devilish elixir. Temptation defeats restraint once he drinks the potent fluid, igniting unbridled vanity and sinful lust for the noblewoman Aurelie.
Sent on a journey to Rome, Medardus encounters Count Viktorin on a precipice and inadvertently pushes his secret half-brother off the cliff. Taking the fallen count's identity at the estate of Baron von R., Medardus plunges into violent passions. He murders Baroness Euphemie and the noble Hermogen when his hypocrisy is threatened. The specter of his surviving double haunts every escape attempt.
Medardus reaches Rome, where his dark brilliance wins him influence among the high clergy and worldly courtiers. Yet worldly success fails to extinguish the horror of his shattered psyche and recurring visions of his monstrous double. Intrigues and moral degradation force him into severe spiritual crisis, eventually breaking his arrogance and driving him back toward repentance.
Returning to his original monastery, Medardus submits to harsh penance to cleanse his tainted soul. The fatal climax arrives when Aurelie prepares to take her holy vows, only to be murdered at the altar by the lunatic double Viktorin. Medardus withstands the ultimate test of temptation and sanity, completing his confession before dying in holy peace.
